2025 Fall Semester – New Student Health Examination Notice

Please read the following descriptions and complete the new student health examination according to relevant regulations such as School Health Act and Communicable Disease Control Act.
Those who fail to complete the health examination in accordance with the regulations, which may affect public safety, will be punished according to the relevant regulations of NTHU.

How to:
1.Print out the NTHU New Students Health Exam Record(attached as below) and bring it to the hospital for health examination.
2.Please choose one of the following options to complete the health examination:

  Off-campus Health Examination
at Cooperative Hospital in Taiwan		 Off-campus Health Examination
at Non-Cooperative Hospital in Taiwan
Hospital Yeezen General Hospital 怡仁醫院 Accessible only district level or above hospitals

  • Transportation: You could Take a train from Hsinchu Station to Yangmei Station in Taoyuan. Then, transfer to the hospital's shuttle bus or take a taxi to the hospital.
Accessible only district level or above hospitals
Time
  • Please complete the process within two weeks before the registration date.
  • Available Hours：
    • Monday to Saturday：08:00 ~ 11:00
    • Monday to Friday：13:30 ~ 16:00
 Only those done after June 1, 2025 at district level or above hospitals are acceptable.
All examination items must match those on the NTHU New Students Health Exam Record. Any missed items must be reexamined.
Fee NT$ 810 (cash only)
Please print out and fill out the NTHU New Students Health Exam Record before visiting Yeezen General Hospital.		 Fees vary by hospital.
We recommend contacting the hospital in advance.

Documents to

  1. Registration Procedure Form

  2. Completed NTHU Health Exam Record (attached as below)with hospital stamp
  3. If no stamp, submit 1 original hospital report with all required items
  • Report must be dated after June 1, 2025
  • Complete the health exam 10–14 days before registration
  • International students may ask hospitals to mail reports to:

Note: Students who apply for leave of absence can postpone the exam. Submit the report when resuming studies.
Examination Notices
  1. If you are ill or have a fever, please postpone the health check.
  2. Blood test, urinalysis, and chest X-ray are included. Fast for at least 6 hours prior (water only).
    • Afternoon exams: breakfast allowed, but still 6 hours fasting required.
    • Bring food and snacks for after the blood test. Juice will be provided.
  3. Do not wear clothes with metallic/sequin buttons or bring valuables. Pregnant students should inform staff and skip X-ray.
  4. Wear shoes that are easy to remove (socks off for weight measurement).
